Blog |Can Poor Posture Cause Dizziness? The Connection Between Your Neck, Posture and Balance

Dizziness is one of those symptoms that can be difficult to describe. Some people feel like the room is spinning. Others feel off balance, lightheaded, unsteady, or almost like their head isn’t keeping up with their body when they move.

There are many possible causes of dizziness, and some of them have nothing to do with the spine. Inner ear disorders, medications, dehydration, blood pressure changes, neurological conditions, cardiovascular problems, and other health issues all need to be
considered.

But there is another potential contributor that doesn’t get as much attention: the neck.

Your cervical spine does more than hold your head up. The muscles and joints of the neck contain sensory receptors that continuously provide your brain with information about the position and movement of your head. When that information doesn’t match what your eyes and inner ear are reporting, some people can experience dizziness or a sense of disequilibrium.

This is often referred to as cervicogenic dizziness.

What Is Cervicogenic Dizziness?

Cervicogenic dizziness describes dizziness or unsteadiness associated with dysfunction or pain originating from the cervical spine.

It is generally not the classic room spinning sensation people associate with vertigo.

Patients are more likely to describe feeling:

  • Unsteady or off balance
  • Lightheaded
  • Disoriented during head movement
  • “Floaty” or disconnected
  • Worse after sitting at a computer for a long time
  • Worse when the neck is particularly stiff or painful
  • Better when their neck symptoms improve

One of the challenges is that there isn’t a single test that definitively proves dizziness is coming from the neck. Cervicogenic dizziness is generally considered after other appropriate causes have been evaluated.

That’s important. I would never assume someone’s dizziness is simply a posture problem without looking at the bigger picture.

What Does Your Neck Have to Do With Balance?

Your brain relies heavily on three systems to understand where your body is in space.

These include your visual system, your vestibular system in the inner ear, and your proprioceptive system.

Proprioception is your body’s internal position sense. It allows you to know where your head, arms, legs, and joints are without having to look at them.

The upper cervical spine contains a particularly high concentration of proprioceptive receptors. These receptors constantly send information to the brain about head position and movement.

Your brain then compares that information with signals coming from your eyes and inner ear.

Normally, those systems work together remarkably well. If the information becomes inconsistent, however, the brain has to reconcile conflicting sensory input.

That sensory mismatch is one proposed mechanism behind cervicogenic dizziness.

How Could Poor Posture Contribute to Dizziness?

This is where posture becomes interesting.

Look at how most of us spend the day. We’re sitting at computers, looking down at phones, driving, working from laptops, and spending hours with our heads positioned in front of our shoulders.

The average adult head weighs roughly 10 to 12 pounds. When the head moves forward relative to the shoulders, the muscles of the cervical spine have to work differently to support it.

Over time, forward head posture can be associated with changes in:

  • Cervical muscle activity
  • Joint loading
  • Neck mobility
  • Muscle tension
  • Head and neck position sense
  • Movement patterns

That does not mean forward head posture automatically causes dizziness. Plenty of people have poor posture without experiencing dizziness at all.

The relationship is more complicated than that.

What it does mean is that altered cervical mechanics can potentially change the sensory information coming from the neck. In someone already experiencing neck pain, stiffness, restricted movement, or impaired cervical proprioception, posture may be one
part of the overall problem.

Why Neck Position Sense Matters

One of the most interesting aspects of chronic neck dysfunction is that the problem isn’t necessarily limited to pain.

Researchers have studied what’s called cervical joint position error, essentially measuring how accurately someone can return their head to a neutral position after moving it.

Some people with neck pain demonstrate impaired cervical position sense. That matters because the brain depends on accurate information from the neck to coordinate movement with the visual and vestibular systems.

Think about what happens when you turn your head.

Your eyes respond.

Your inner ear responds.

Your cervical joints and muscles respond.

Your brain has to integrate all three sources of information almost instantly.

When the signals don’t agree, the result may be more than a stiff neck.

Can Forward Head Posture Affect the Nervous System?

I think this question needs to be discussed carefully because there are a lot of exaggerated claims online.

Your nervous system is constantly receiving sensory information from muscles, joints, vision, the vestibular system, skin, and other tissues. Changing the position and movement of the head and neck changes some of that sensory input.

That is basic neurophysiology.

What we cannot responsibly say is that every case of dizziness, brain fog, anxiety, fatigue, or another neurological symptom is caused by a spinal misalignment. Human physiology isn’t that simple.

What interests me clinically is the patient who has several findings occurring together:
chronic forward head posture, significant neck tension, restricted cervical movement, headaches, poor position sense, and dizziness associated with neck movement.

That pattern deserves a closer look at the cervical spine.

Dizziness vs. Vertigo: They Aren’t the Same Thing

Patients often use “dizziness” and “vertigo” interchangeably, but they’re not necessarily describing the same sensation.

Vertigo typically refers to an illusion of movement. You may feel like you are spinning or
the room is moving even though you’re standing still.

Dizziness is broader. It can include lightheadedness, imbalance, disorientation, or feeling unsteady.

That distinction can help narrow down the potential cause.

For example, benign paroxysmal positional vertigo, commonly called BPPV, involves the vestibular system of the inner ear and frequently produces a distinct spinning sensation associated with certain head positions.

Cervicogenic dizziness is more commonly described as disequilibrium, unsteadiness, or disorientation associated with neck pain or movement.

How Do You Know If Dizziness Is Coming From Your Neck?

There isn’t a simple home test that can reliably answer this.

I become more interested in a cervical component when someone has dizziness along with findings such as:

  • Neck pain or stiffness
  • Reduced cervical range of motion
  • Frequent headaches
  • Symptoms that change with neck position
  • A history of whiplash or neck injury
  • Significant postural changes
  • Chronic upper trapezius or suboccipital tension
  • Symptoms that seem to increase as neck discomfort increases

Even then, correlation doesn’t prove causation.

The goal of an examination is to determine whether the neck appears to be contributing while also recognizing when the symptoms require evaluation outside of a chiropractic office.

When Dizziness Should Be Taken Seriously

Dizziness should never automatically be blamed on posture.

New, severe, persistent, or unexplained dizziness warrants appropriate evaluation, particularly when it occurs with neurological or cardiovascular symptoms.

Seek urgent medical evaluation for dizziness associated with symptoms such as:

  • New difficulty speaking
  • Facial drooping
  • Sudden weakness or numbness
  • Severe or unusual headache
  • Loss of consciousness
  • New double vision or significant visual changes
  • Difficulty walking that develops suddenly
  • Chest pain
  • Significant shortness of breath
  • Symptoms following a significant head or neck injury

Those are not symptoms to self-diagnose.

How I Evaluate Neck Pain and Dizziness

When someone comes into Crown City Chiropractic & Wellness with dizziness and neck complaints, I don’t start with an adjustment. I start by trying to understand what we’re dealing with.

The history matters tremendously. When did the dizziness start? What does it actually feel like?

Does moving the neck reproduce it? Is there neck pain? Was there an injury?

Are there headaches? Have there been visual changes? What other medical conditions or medications need to be considered?

Depending on the individual, the examination may include:

  • Cervical range of motion
  • Neurological testing
  • Orthopedic examination
  • Postural assessment
  • Balance and functional testing
  • Evaluation of cervical movement
  • Digital X rays when clinically indicated

If the history or examination suggests that the dizziness may be coming from something outside the musculoskeletal system, appropriate referral comes first.

Can Chiropractic Care Help Cervicogenic Dizziness?

There is evidence suggesting that manual therapy may help some patients diagnosed with cervicogenic dizziness, particularly when dizziness occurs alongside neck pain and cervical dysfunction. The research is promising, but it isn’t strong enough to claim that
chiropractic treatment is appropriate for every person experiencing dizziness.

That’s why diagnosis matters.

If the examination suggests a significant cervical mechanical component, care may involve more than an adjustment. Depending on the patient, I may address spinal mobility, posture, cervical movement, muscular function, balance, ergonomics, and
specific exercises.

The goal isn’t to & “treat dizziness” as an isolated symptom. It’s to identify the dysfunction we can actually measure and address.

What Can You Do About Poor Posture?

If you spend most of your day sitting, small changes add up.

Start with your workstation. Your monitor should be positioned so you’re not constantly looking downward. Bring your phone closer to eye level instead of repeatedly dropping your head toward your chest. Change positions throughout the day rather than trying to
hold one supposedly perfect posture for eight hours.

Movement matters too.

Regularly stand up, walk, move your shoulders, rotate your upper back, and give your neck a break from sustained positions. Strengthening the muscles of the upper back and improving thoracic mobility can also reduce some of the mechanical demand placed on the cervical spine.

The goal isn’t perfect posture every second of the day. The goal is a body that can move well and tolerate the positions your life requires.

The Bottom Line

Can poor posture cause dizziness?

It may contribute in certain people, particularly when dizziness occurs alongside neck pain, stiffness, altered cervical mechanics, or impaired position sense. But dizziness has many possible causes, and posture should never automatically be
assumed to be the source.

The connection between the cervical spine and balance makes sense anatomically and neurologically. Your brain continuously integrates information from your eyes, inner ear, and the sensory receptors of your neck. When cervical function is impaired, that
information may become less reliable.

If you’re experiencing recurring dizziness along with neck pain, headaches, stiffness, or significant postural changes, it’s worth figuring out why.

People commonly describe cervicogenic dizziness as unsteadiness, disequilibrium, lightheadedness, or feeling disoriented rather than experiencing true room spinning vertigo. Symptoms frequently occur alongside neck pain or stiffness.

Can forward head posture cause dizziness?
Forward head posture changes the mechanical demands placed on the cervical spine and surrounding muscles. It may be one contributing factor in people with cervical dysfunction, but forward head posture by itself does not prove the cause of dizziness.
Can tight neck muscles cause dizziness?

Neck muscle dysfunction may affect cervical proprioception, which helps the brain determine head position. In some patients with neck dysfunction, this may contribute to dizziness or imbalance. Other causes of dizziness should still be considered.

Can neck problems affect your balance?
Yes. The cervical spine contains sensory receptors involved in proprioception. Information from these receptors is integrated with visual and vestibular information to help control balance and orientation.

How do I know if my dizziness is from my neck or inner ear?
The symptoms can overlap, so this usually requires an appropriate examination. True spinning vertigo, particularly when triggered by specific positional changes, may suggest a vestibular cause. Cervicogenic dizziness more commonly occurs with neck
pain, stiffness, restricted movement, or a history of cervical injury.

Can a chiropractor help with dizziness?
Chiropractic or other conservative manual care may be appropriate for some people with cervicogenic dizziness when cervical musculoskeletal dysfunction is present. Dizziness from vestibular, neurological, cardiovascular, medication related, or other
causes requires treatment directed toward that cause.

Should I get my neck checked if I have dizziness and headaches?
Recurring dizziness accompanied by neck pain, stiffness, headaches, or restricted movement warrants evaluation. The appropriate provider depends on the symptoms and examination findings. Sudden or severe neurological symptoms require urgent medical assessment.

Can looking down at my phone cause dizziness?
Prolonged phone use can contribute to sustained forward head positioning, cervical muscle fatigue, and neck discomfort. Whether this causes dizziness depends on the individual. If symptoms repeatedly occur after prolonged neck positioning, cervical function may be worth evaluating.

Is dizziness from the neck dangerous?
Cervicogenic dizziness itself is not generally considered a dangerous condition, but dizziness can also occur with serious neurological, cardiovascular, and other medical conditions. New or unexplained dizziness should not be self-diagnosed.
